Primary Care: Ambulatory care for acute and chronic illnesses for adults and children. Referrals: a) Specialist and sub-specialist for definitive diagnosis and prescription. b) For hospitalization as needed c) Public Health Nurse or other appropriate agency. HIV/AIDS Primary Care: Evaluation, treatment and follow-up, provided as part of expanded sexually transmitted disease services.	Services will be offered to those individuals with a confirmed HIV+ test, or who are exhibiting symptoms associated with an HIV infection, or people concerned about the possibility of having symptoms. Immunization Registry The California Immunization Registry (CAIR) oversees the activities of the Inland Empire Immunization Tracking System (IEITS) in Riverside County. The CAIR is a confidential, computerized information system that manages information pertaining to childhood immunizations. The Registry will provide better access to information for records that may have been lost; prevent duplicate vaccinations; and through its reminder/recall system ensure more timely immunizations. Administrative and Case Management Services California Children Services (CCS) is a statewide program of specialized medical care and rehabilitation for children with special health care needs. CHILD HEALTH AND DISABILITY PREVENTION PROGRAM (CHDP)
CHDP-provides well child checkups which include: History and physical examination, dental assessment, nutritional assessment, vision screening, hearing screening, blood and urine testing, TB testing, immunizations, health education, referral for diagnosis and treatment.	Children from birth to 21 years of age who have Medi-Cal and are not enrolled in a Medi-Cal managed care program. In-home services provided by Public Health Nurses include assessment, guidance, counseling, teaching, intervention, referral and family case management in the categories listed below: Child health, adult/senior health, perinatal, postpartum, family planning, parenting education, (in-home and also group classes in various cities throughout the County), high risk pregnancy, high risk infant, grief/loss, child abuse prevention, adult/elder abuse, communicable disease, tuberculosis, infant death, drug exposed infant, home birth/newborn screening follow-up, homeless, domestic violence, teen parenting, and wraparound services. WRAPAROUND PROGRAM
Public Health Nurses work in collaboration with the Department of Mental Health, Social Services, Probation and Education to ensure that the health related needs of families are identified and addressed through available community resources. In this partnership the Public Health Nurse provides support individualized to each family’s unique strengths and needs promoting health, safety and permanence in the home, school and community.	Children are referred to the Wraparound Program from the Probation Department or the Mental Health Case Management Department. Before they can be referred, these children need to be in placement or at risk for a high level group home placement. They also need to have Medi-Cal and a family to go home to. NURSE FAMILY PARTNERSHIP PROGRAM
Public Health Nurses provide individualized care to meet the needs of first time moms and their babies. This includes monitoring pregnancy for complications, information on what to expect at the time of delivery and how to care for herself and child at home. The mother and child are visited in their home regularly until the child reaches 2 years of age.	First-time, high-risk pregnant females with low income. Fees are not charged. Nutrition Network Promotes healthy eating and a physically active lifestyle among low income Californians by using social marketing techniques. Promotes protective behaviors that help prevent cancer and other chronic diseases, educational activities to promote healthy eating behaviors and physical activity. 5 A DAY Power Play Campaign Promotes the increase of consumption of fruits and vegetables in children. To decrease incidences of chronic diseases. Provides educational activities to increase children's consumption of fruits and vegetables are organized through community channels including schools, community youth organizations, farmers markets, and retail. Childhood Asthma Program: Provides services for children diagnosed with asthma and their parents. The Medical Marijuana Identification Card (MMIC) Program was designed to provide patients an ID card to document recommendation from their physician to use marijuana for medicinal purposes. Community Outreach Services provides essential information to the community, CHA employees and businesses regarding the Department of Public Health. Through annual health fairs and events, we establish community relationships to better serve the needs of Riverside County residents. This office also works as a liaison with public and private agencies to promote health activities and outreach opportunities. Our Department of Health Auxiliary provides support to our various Agency programs through volunteer and fundraising activities. Financial support includes match donations, purchase of non-budgeted items (i.e., equipment for employee break rooms, transportation vouchers, and other client assistance), and items of critical need. VOLUNTEER SERVICES
Volunteer Services welcomes men and women who are interested in volunteering to assist in the delivery of health services within our Department of Public Health programs. Volunteers are given the opportunity to utilize their skills, time and energy for the overall good of the community. By donating their time and talent, volunteers enhance quality and availability of public health services. A six-month commitment is requested.	Open to anyone 16 years of age and older.
