The Public Health Nurse can assist you:
- In finding a source of medical care for you or a family
member
- In finding community resources for you and your family
- In determining health needs for you or a loved one
- By providing education and counseling regarding you
or a family members health concerns
- By teaching positive health practices including nutrition
and medication usage
- By offering counseling and resources if you have lost
a loved one
- If you are exposed to a communicable disease

This is a multi-disciplinary team focusing on the unique challenges
of grandparents raising their grandchildren.

Older Adult System of Care Committee
The purpose of the committee is to collaborate among care providers
to co-ordinate and improve access for seniors to obtain health, mental
health, and substance abuse services.

- CARETEAM
(Curtailing Abuse Related to the Elderly)
This is a multi-disciplinary team that works on specific complex senior
cases. The team focuses on elder abuse cases and fiduciary abuse. The
role of the PHN is to provide assistance and follow-up.
