Public Health Nursing

SENIOR SERVICES

The Public Health Nurse can assist you:

    Senior Services
  • In finding a source of medical care for you or a family member
  • In finding community resources for you and your family
  • In determining health needs for you or a loved one
  • By providing education and counseling regarding you or a family members health concerns
  • By teaching positive health practices including nutrition and medication usage
  • By offering counseling and resources if you have lost a loved one
  • If you are exposed to a communicable disease

Senior Services that PHN's are involved with:

  • Grandparents Raising Grandchildren Task Force
    This is a multi-disciplinary team focusing on the unique challenges of grandparents raising their grandchildren.

  • Senior Resources Older Adult System of Care Committee
    The purpose of the committee is to collaborate among care providers to co-ordinate and improve access for seniors to obtain health, mental health, and substance abuse services.

  • CARETEAM (Curtailing Abuse Related to the Elderly)
    This is a multi-disciplinary team that works on specific complex senior cases. The team focuses on elder abuse cases and fiduciary abuse. The role of the PHN is to provide assistance and follow-up.
